自测血糖老不准？小心测量误差

如今许多糖尿病人在家都备有血糖仪，方便监测自身血糖的浓度。但常常有患者发现在家自测的血糖值与医院的化验单结果不一致，后者的数值通常偏高，这大大打击了糖友们的控糖信心。患有2型糖尿病的孙大爷就遇过这种情况。他用儿子给他买的快速血糖仪测血糖，但每次到医院复查测血糖后都发现医院的值要比在家测的略高一些。孙大爷很担心，也很不解。
专家表示，这是由于样本类型不同而引起的差异。一般来讲，患者在医院抽血测的是静脉血浆血糖，即过滤掉红细胞、白细胞等血细胞后血浆里的血糖浓度，而家用血糖仪测的一般是指尖毛细血管的动脉全血血糖。两者的差异取决于血细胞在血液中的比例，血细胞比例越高，差异越大。总体上，血浆较全血高出约11%。有些型号的血糖仪将试纸进行改进，其多孔膜可分离血细胞，从而使得结果与血浆接近，减少差异。
哪些因素可致误差？
如上述情况，有些糖尿病人以为家用血糖测量仪可以取得跟医院检验科一致的数据，而事实并非如此。那么还有哪些因素能导致误差，患者在家里自测血糖又需要注意哪些地方？
1. 采血时间造成误差。一般在空腹8小时以上时两者血糖值无大差异，但进食后毛细血管血（大致相当于动脉血）比静脉血血糖值高。这是因为，进食后肠道吸收的葡萄糖经腔静脉回心后流到动脉，动脉血经过毛细血管与组织代谢消耗了葡萄糖后，才进入静脉。一般认为进食后此差异与血浆、全血差异两者抵消，采指血与抽血的血糖结果几乎相等。
2. 血糖仪保养及使用不当。一般认为，家用血糖仪的误差如在15%以内，不会影响治疗措施的改变，故是可以接受的。但有的患者所测的结果相差达31%，主要原因可能是对血糖仪的清洁维护不当。血糖仪在放置过程中可能会被落尘污染，特别是检测时不小心涂抹在机器上的血液都可影响测试结果，因此一定要定期检查保养。
3. 未按说明书规定操作。如未校正试纸代码，试纸保存可受温度、湿度、光线、化学物质等的影响而发生变化，造成检测误差。
患者在家自测血糖需要注意几方面：使用前检查血糖仪和试纸是否匹配。注意保质期。过期的试纸不能再用。受潮后试纸也必须更换。采血时要规范，针刺后不可挤压。
糖尿病是一个需要自我管理的疾病，而自测则是帮助患者了解自己情况的必需手段，但自测误差带来的过度治疗或治疗不足都能耽误病情。因此患者要正确监测血糖，有效地控制糖尿病。
